While submitting your form, you will also need to provide us with a copy of a current South Carolina driver's license or state-issued ID.
Once your information is received, we will email you your appointment details, which will include a link to your virtual evaluation. When it comes time for your appointment to begin, you will be connected to one of our physicians who will review your patient records, discuss medical marijuana treatment, and upon receiving approval, write your certification.
